Thank you for supporting our school as a volunteer!

Volunteers help create a safe, welcoming, and inclusive place for students to learn. To support this, all volunteers must follow district safety guidelines.

General School year forms and fees

Each year, New Westminster Schools asks families to complete a few important consent forms so we can support student safety and participation in school activities.

These forms may be completed through SchoolCash Online and include:

  • Emergency release
  • School and district codes of conduct
  • Digital Technology and Responsible Use Agreement
  • Walking field trip consent
  • Media, photo, and video release
  • G Suite for education
  • BC School Fruit, Vegetable and Dairy Nutrition Program

From time to time, parents are required to pay for activities or services provided by our schools, such as class trips, sports activities, and yearbooks. Fees may also be paid through SchoolCash Online.

Medical Intervention forms

If your child has a serious medical condition (such as asthma, diabetes, epilepsy, or a severe allergy), parents must complete a medical intervention form. This form includes details about the condition, required medication, and what to do in an emergency.

A medical intervention form is also needed if the school is asked to store or give routine medication, like ADHD medicine, tylenol or advil.

Staff cannot give any medication, including over-the-counter items, without a signed form and medication provided by the family.

Forms must be updated as needed and at least once each school year. Updated versions for 2025–2026 school year are now available. Please review the list below to see which may apply to your child; and download as needed.

  • SD40 Medical Intervention Administrative Procedure (this is not a form, but a document regarding Medical Alerts in Schools, why this information is collected, and how it is used to support your child)
  • Medical Intervention form (MANDATORY for all medical alerts and office-stored medication not specific to anaphylaxis, diabetes, or seizure disorders.)
  • Anaphylaxis Procedure Plan
  • Administration of Medication Record form (for the school to fill out as regular medication is dispensed)
  • Diabetes support plan
  • Seizure action plan
