School year forms and fees
Each year, New Westminster Schools asks families to complete a few important consent forms so we can support student safety and participation in school activities.
These forms may be completed through SchoolCash Online and include:
- Emergency release.
- School and district codes of conduct.
- Digital Technology and Responsible Use Agreement.
- Walking field trip consent.
- Media, photo, and video release.
- G Suite for education.
- BC School Fruit, Vegetable and Dairy Nutrition Program.
From time to time, parents are required to pay for activities or services provided by our schools, such as class trips, sports activities, and yearbooks. Fees may also be paid through SchoolCash Online.

Medical Forms
MEDICAL INTERVENTION FORM ~ fill out this form and return it to the office if your child has a medical condition and/or requires medication at school.
ANAPHYLAXIS PROCEDURE PLAN ~ fill out this form and return it to the office if your child has an allergy that could cause an anaphylactic reaction and requires a single-dose auto-injector (EpiPen) to be kept at school.
If your child requires a Type 1 Diabetes Action Plan for Diabetes Management or a Seizure Action Plan for Seizures, please contact the school office.
Volunteer form
Thank you for supporting our school as a volunteer.
Volunteers help create a safe, welcoming, and inclusive place for students to learn. To support this, all volunteers must follow district safety guidelines.
Before volunteering, please review the following:
